A: These templates are starting points. Have your church’s attorney review them for compliance with your state’s laws. Specific requirements vary by state, especially for background checks, medical authorizations, and facility use agreements.

Q: Do we really need background checks for all volunteers?
A: Yes. Every volunteer who works with children or youth should have a background check. It protects children, volunteers, and the church. Many insurance providers now require it.

Q: How long should we keep completed forms?
A: Volunteer applications and background check authorizations: at least 3 years after the volunteer leaves. Permission slips: at least 3 years after the event. Medical forms: review annually, keep current year plus one prior year. Donation receipts: keep for 7 years for tax purposes.

Q: Can I modify these forms for my church?
A: Yes. Customize them to fit your church’s needs. Add your church’s name, logo, and contact information. Adjust sections to match your church’s specific policies and state requirements.

Q: Do I need a facility use agreement if we only let other church groups use our building?
A: Yes. Even for church-to-church use, a written agreement protects both parties. It clarifies expectations for setup, cleanup, damage, cancellation, and insurance. It is much easier to have the conversation before a problem than after one.

Q: How should we handle donation receipts for non-cash contributions?
A: The donor determines the fair market value of non-cash gifts. Your receipt should describe the item but not assign a value. For gifts of $500 or more, the donor must file IRS Form 8283. When in doubt, consult a tax professional familiar with non-profit regulations.
