Most small churches operate without the basic forms they need. Not because they do not care about organization, but because nobody has ever told them what forms they need or where to get them.
Here is a practical guide to the essential ministry forms every small church should have, organized by category, with free templates available for each one.
Category 1: Child Safety Forms
Child safety forms are not optional. They protect children, protect volunteers, and protect the church from liability.
- Background check authorization form. Every volunteer who works with children must complete a background check. This form authorizes the check and collects the necessary information.
- Volunteer application form. Collects basic information about volunteers, their experience, and their references.
- Child check-in and check-out form. Documents who dropped off a child and who is authorized to pick them up.
- Medical information and permission form. Collects allergy information, medical conditions, and emergency contact information for each child.
- Incident report form. Documents any injury, illness, or concerning incident involving a child in your care.
Category 2: Financial Forms
- Offering count sheet. Documents the amount counted, who counted it, and when. Requires two signatures.
- Expense reimbursement form. Documents ministry expenses submitted for reimbursement, with receipts attached.
- Annual donation receipt. Sent to every donor at year-end for tax purposes. Required by the IRS for donations of $250 or more.
- Benevolence request form. Documents requests for financial assistance, the need, and the decision made.
- Budget request form. Used by ministry leaders to request budget allocations for the coming year.
Category 3: Membership Forms
- Membership application form. Collects basic information about prospective members and their faith background.
- Membership transfer form. Used when a member transfers from another church. Requests a letter of transfer from the previous church.
- Membership removal form. Documents the process and reason for removing a member from the membership roll.
Category 4: Event and Ministry Forms
- Event permission slip. Used for any off-site event involving minors. Collects parental permission and emergency contact information.
- Facility use request form. Used when outside groups request to use the church building. Documents the group, the purpose, and the terms of use.
- Volunteer commitment form. Documents a volunteer’s commitment to a specific role, including the time commitment and expectations.
- Ministry leader report form. Used by ministry leaders to report on their ministry’s activities, attendance, and needs.
Category 5: Pastoral Care Forms
- Pastoral visit log. Documents pastoral visits, who was visited, when, and what was discussed. Helps ensure no one falls through the cracks.
- Prayer request form. Collects prayer requests from congregation members for the pastoral prayer list.
- Counseling intake form. Collects basic information before a pastoral counseling session, including the presenting concern and relevant background.
